I think it was my dad who noted that in naval engagements, the captains and admirals invariably followed whatever doctrine had been drilled into them, regardless of whether it made sense:
The Bismark ran away when it should have finished off the Prince of Wales, but preserved the ship.
The Prince of Wales and Repulse should have avoided engagement with the IJN and remained a force in being, but sailed forth to engage the IJN invasion force and was destroyed.
And the USN, while it did not have a solid surface warfare doctrine at the start of the war, had several competing factions internally, and their champions all had cases where they went to their default baseline when tactically and strategically it would have been better for them to go to other tactics. Spruance going turtle when he needed to go aggressive. Halsey's Bull Run when he should have gone turtle.
And these are the top of the top performers operating in areas where they all had extensive experience. Yet they all dropped back to their default trained reaction under stress. That's just how people work. All you can do is train your reflexive response, and plan around doing complex reflective work when you're under a lot of stress.
